Shippers Export Declaration:
This is the official U.S. Department of Commerce form used for compiling U.S. export statistics and export control. The form is required for all shipments of individual items valued at $2,500 or more ($500 parcel post shipments).
The forms, which are approved by the Office of Management and Budget, come in three formats: vertical, horizontal and faxable. Two versions are printed on goldenrod colored paper; the faxable form is printed on white paper with nonphoto blue ink. NOTE: As of January 1991, most exports to Canada did not require a Shipper's Export Declaration form.
