Department of the Treasury/U.S. Customs Form is used to expedite free entry or entry at a reduced rate on foreign-made personal articles that are taken abroad. These articles are subject to duty each time they are brought into the United States unless there is acceptable proof of prior possession. It is used by individuals who travel abroad and is filled out at the port of entry/departure.
